This application is a request to be in the tournament and does
not imply acceptance until notified in writing by the Atlanta Fire
Hockey Club Tournament Director.
Contact information for Tournament Host Hotels will be provided once
your application is accepted. The Cooler would prefer if your team stayed at a host hotel.
Full payment is due to reserve a spot for your team. $250 of the fee is
non-refundable. Refund requests must be made by November 30, 2009 in writing.

Checks, money orders
(made to 'ATLANTA FIRE PEEWEE MAJOR') or credit card information with total fees due must be sent with
application. You can also fax your registration to The Cooler at
770-649-6572. Visa, MasterCard, Discover & American Express are accepted.
